Only use this form to claim 2023 Customer Loyalty rebates purchased through a Participating Authorized Dealer. A copy of the dealer’s sales invoice referencing the product(s) must be attached below. MTI Customer Loyalty Rebate Claim Form must be submitted by November 15, 2023.

Qualifying products are the MTI chair model numbers 840, 830, 829, 721, 710, 550, 529, 529W, 528, 464, 463, 461 and 403. Limited time offer. The 2023 Customer Loyalty Promotion is only valid for purchases made from July 10, 2023 to September 30, 2023, with shipment and invoice by November 15, 2023. Invoice date must be from July 10, 2023 to November 15, 2023. The Rebate is only valid for current MTI Customers in the USA. This rebate is valid on qualifying products purchased by a physician or medical/dental facility who are the original purchaser of an MTI exam or procedure chair before July 10, 2023 and who is purchasing a new, qualifying product through a Participating Authorized Dealer. This offer is not valid for products ordered before July 10, 2023 and is not valid with any other promotions, incentives or contracted pricing agreements. The rebate must be claimed through the online MTI 2023 Customer Loyalty Rebate Form by November 30, 2023. Claimant must submit the serial number of at least one MTI exam or procedure chair invoiced before July 10, 2023, as evidence that they are a customer of MTI, as well as the serial number(s) of the new qualifying product(s) upon which the claim is based, along with the applicable dealer invoice. The rebate check will be mailed within 90 days after claim validation.
This offer is a reduction in price or discount on the purchased product(s). Purchaser is obligated to: (i) disclose the amount of this price/discount in claims or cost reports for programs that require such disclosure, such as; reimbursement to programs for the US federal healthcare system, Medicare or Medicaid; and (ii) upon request, provide the discount documentation to representatives of the US Secretary of the Department of Health and Human Services and other state agencies as required. MTI will provide additional information to the Purchaser as necessary to meet reporting or disclosure requirements.
