Rebate is four hundred US dollars to for each new MTI chair model number 840, 830, 829, 721, 550, 529, 529W, 464, or 463, or two hundred US dollars to for each new MTI chair model number 710, 528, 461, or 403, or five hundred US dollars for each new MTI chair and cabinet combination for chair model number 840, 830, 829, 464, 463, 461, or 403 and MTI TC Series Cabinet model number TC100 or TC75. Chair and cabinet combination must be purchased and delivered at the same time. Customer may quality for only one rebate per chair serial number or chair and cabinet serial number combination.
The 2023 Fall Promotion is only valid for orders placed from October 1, 2023 to December 31, 2023, with shipment and invoice by February 15, 2024. Invoice date must be from October 1, 2023 to February 15, 2024. The Rebate is only valid for customers in the USA. This rebate is valid on qualifying products purchased by a physician or medical/dental facility who is purchasing a new, qualifying product through a Participating Authorized Dealer. This offer is not valid for products ordered before October 1, 2023. Cancelling a current order and reordering to fit within the promotion timeline will void the order for qualification of the rebate. This offer is not valid with any other promotions, incentives or contracted pricing agreements. The rebate must be claimed through the online MTI 2023 Fall Promotion Form by February 29, 2024. The rebate check will be mailed within 90 days after claim validation.
This offer is a reduction in price or discount on the purchased product(s). Purchaser is obligated to: (i) disclose the amount of this price/discount in claims or cost reports for programs that require such disclosure, such as; reimbursement to programs for the US federal healthcare system, Medicare or Medicaid; and (ii) upon request, provide the discount documentation to representatives of the US Secretary of the Department of Health and Human Services and other state agencies as required.
