¹Terms & Conditions: Limited time offer. Rebate is seven hundred fifty US dollars for each new MTI chair model number 840, 830, 829, or 550; five hundred US dollars for each new MTI chair model number 529 or 529W; four hundred US dollars for each new MTI chair model 721 or 464; two hundred US dollars for each new MTI chair model number 710, 528, 463, 461, or 403. Alternatively, rebate is six hundred US dollars for each new MTI chair and cabinet combination for any chair model above when purchased with MTI cabinet model number TC100. Chair and cabinet combination must be purchased and delivered at the same time. Customer may quality for only one rebate per chair serial number or chair and cabinet serial number combination.
The 2024 Winter Promotion is only valid for orders placed from January 15, 2024 to March 31, 2024, with shipment and invoice by May 15, 2024. Invoice date must be from January 15, 2024 to May 15, 2024. The Rebate is only valid for customers in the USA and Canada. This rebate is valid on qualifying products purchased by a physician or medical/dental facility who is purchasing a new, qualifying product through a Participating Authorized Dealer. This offer is not valid for products ordered before January 15, 2024. Cancelling a current order and reordering to fit within the promotion timeline will void the order for qualification of the rebate. This offer is not valid with any other promotions, incentives, or contracted pricing agreements. The rebate must be claimed through the online MTI 2024 Winter Promotion Form by May 31, 2024. The rebate check will be mailed within 90 days after claim validation.
This offer is a reduction in price or discount on the purchased product(s). Purchaser is obligated to: (i) disclose the amount of this price/discount in claims or cost reports for programs that require such disclosure, such as; reimbursement to programs for the US federal healthcare system, Medicare or Medicaid; and (ii) upon request, provide the discount documentation to representatives of the US Secretary of the Department of Health and Human Services and other state agencies as required.
