¹Terms & Conditions: Limited time offer. Rebate is $500 (five hundred US dollars) for each new MTI power chair and TC series cabinet combination for any power chair model when purchased with an MTI TC Series cabinet model number TC100 or TC75. Chair and cabinet combination must be purchased and delivered at the same time. Customer may qualify for only one chair and cabinet serial number combination.
The 2025 Fall Promotion is only valid for orders placed from September 1, 2025 to November 30, 2025, with shipment and invoice by February 28, 2026. The Rebate is only valid for customers in the USA and Canada. This rebate is valid on qualifying products purchased by a physician or medical/dental facility who is purchasing a new, qualifying product through a Participating Authorized Dealer. This offer is not valid for products ordered before September 1, 2025. Cancelling a current order and reordering to fit within the promotion timeline will void the order for qualification of the rebate. This offer is not valid with any other promotions, incentives, or contracted pricing agreements. The rebate must be claimed through the online MTI 2025 Fall Promotion Form by March 15, 2026. The rebate check will be mailed within 90 days after claim validation.
This offer is a reduction in price or discount on the purchased product(s). Purchaser is obligated to: (i) disclose the amount of this price/discount in claims or cost reports for programs that require such disclosure, such as; reimbursement to programs for the US federal healthcare system, Medicare or Medicaid; and (ii) upon request, provide the discount documentation to representatives of the US Secretary of the Department of Health and Human Services and other state agencies as required. MTI will provide additional information to the Purchaser as necessary to meet reporting or disclosure requirements.
